WAIVER / RELEASE FORM:
In consideration for participation of this league event, I agree to assume all risks and release, indemnify, and hold harmless Champions Flag Football Association (“CFFA”), its shareholders, officers, directors and employees and/or all others having an interest in the CFFA from all liabilities, negligence, causes of actions, claims, demands, changes and damages of every kind which may arise out of my participation in this league event. I will play under control within the rules of the game and to the best of my ability. I will avoid causing injury to myself and others using this facility.
NOTICE OF WARNING:
AGREEMENT:
I have read and understand the waiver and release of liability on this form. I have also read and agree to abide by the published rules of the CFFA.
I consent to the use by CFFA of my/our team photographs, videos, and new media, and I acknowledge and agree that CFFA reserves the sole and exclusive right to use any such photographs or videos for advertising and instructional purposes, without cost or charge.

Champions Flag Football Association Waiver and Release of Liability
This is a binding legal agreement.
As a participant in the programs, activities, and events of the CFFA, the undersigned acknowledges and agrees to the following terms.
DISCLAIMER:
Champions Flag Football Association, its directors, officers, members, employees, volunteers, officials, participants, agents, sponsors, owners/operators of facilities are representatives of (the “Organization”) are not responsible and cannot be held liable for any injury, damage or loss of any kind suffered by a participant during, or as a result of, any program, activity or event, caused in any manner whatsoever including, but not limited to, the negligence of the Organization.
DESCRIPTION OF RISKS:
In consideration of my participation as a Participant in such programs, activities, and events. I hereby acknowledge that I am aware of the risks and hazards associated with or related to flag football and assume all such risks.
The risks and hazards of flag football include, but are not limited to injuries from:
Executing strenuous and demanding physical techniques;
Grass, turf and other surfaces including bacterial infections and rashes;
Falls to the ground due to uneven or irregular terrain or surfaces;
Collisions with goal posts and flag football equipment;
Failure to properly use any piece of equipment or from the mechanical failure of any piece of equipment;
Spinal cord injuries which may render me permanently paralyzed;
Extreme weather conditions which may result in heatstroke, sunstroke or hypothermia;
Contact, colliding or being struck by other participants, spectators, equipment or vehicles;
Vigorous physical exertion and strenuous cardiovascular workouts;
Exerting and stretching various muscle groups; and
Travel to and from competitive events and associated non–competitive events which are an integral part of the Organization’s activities.
Furthermore, I am aware - That injuries sustained in flag football can be severe;
That I may come into close contact with other participants, including the possibility of accidental and unexpected contact;
That I may experience anxiety while challenging myself during the activities;
That my risk of injury is reduced if I follow all rules adopted; and
That my risk of injury increases as I become fatigued.
RELEASE OF LIABILITY:
In consideration of the Organization allowing me to participate as a participant, I agree:
To assume all risks arising out of, associated with or related to my participation;
To be solely responsible for any injury, loss or damage that I might sustain while participating; and
To release, indemnify and save harmless the Organization from all liability for any and all causes of actions, claims, demands, changes, costs and damages of every kind which may arise out of my participation in activity, including in respect of any action that may have been caused by the negligence of the Organization.
DISPUTE RESOLUTION:
This Release shall be governed by and construed for all purposes by the laws of the Province of Ontario, without regard to any conflict of law principles. This Release supersedes any and all previous oral or written promises or other agreements.
TERM:
This waiver and release of liability shall remain in effect for the duration of my participation in the league during the 2026 CFFA league year.
